Ministry of Health and Family Welfare Terms of application:
- Attracted candidates have to submit application to own district’s Civil Surgeon.
- Attracted candidates have to write own name, mother’s name, father’s/husband’s name, own district, age, nationality, present address, permanent address, religion, name of post, date of birth, educational qualification & experience (if have) & also own signature in application form.
- The Last date of application receive 3rd March (on Tuesday) 2015.
- Incomplete & defective application won’t be allowed.
- For application charge Attracted candidates have to pay 100 Taka for 3rd class posts & 50 Taka for 4th class posts.
- Attracted candidates won’t get any TA/DA bill from authority.
- Attracted candidates have to bring written exam’s admit card & main certificate with them in viva/practical test.
- Directorate General of Health Services (DGHS) authority has the right to increase or decrease any posts & Addition, Correction & change anything about the job.
